Human World
This Digimon human-world is exactly like a modern day (2000) Japan.
Digital World
The Digital World is where the actual Digimon come from. It was created by humans to test AI programs and play video games, and has sustained itself, and even grown. Looking at the Digital World on a normal computer will only give you strings upon strings of complex binary, spelling out in 1's and 0's its core coding. But the actual Digital World that exists is a beautiful and wild place, with gorgeous nature, mind-boggling places, and free-willed thinking beings called Digimon.

As Games
Digimon, much like in our world, exist as hand-held games and anime.
As Creatures
Digimon, which stands for Digital Monster, exist mostly in the Digital World. They hatch from digi-eggs, or digitamas, and go through an evolutionary aging process that changes their powers, appearence, and sometimes their personality. Some are feral, like animals, but most are intelligent, and interact like humans. Almost all Digimon spend their life fighting to become stronger, or just because they enjoy it.
When Digimon fight and one loses, the winner has the option of loading their opponent's data. Loading their data is comparable to absorbing their soul, and it affords the winner all of the loser's strength. If the winner does not load their data, the data will disperse and eventually resettle into a digitama, and hatch into the exact same Digimon. But if the data is completely destroyed, their essence dies.
Digimon are classified into three main types; data, vaccine, or virus. Although it's not a constant, virus types are seen as "bad," vaccine as "good," and data as the middle ground. There's also the variable type, which is a kind of hybrid, and unknown types.
Digivolution
High-level Digivolution is usually only possible with Tamers, or Partners; humans who have joined with "Chosen" Digimon for a common goal or to form a severe bond. Most Digimon can naturally evolve to the Champion level over the course of their lifetime, while Chosen Digimon can digivolve quickly and frequently with the help of a Tamer/Partner.
The natural levels of Digivolution are:
1. Fresh, or Baby, form, which is right out of the egg.
2. In-Training (sometimes known as Baby 2 form). Small forms which are usually round and have only the most basic attacks.
3. Rookie (or child form). The basic form. After high-level Digivolution, the Digimon will return to this form.
4. Champion (or adult form). This is a natural Digivolution.
5. Ultimate (or perfect form). This form can only be reached with Blue Cards or Crests, things that are only available to Partners or Tamers.
6. Mega (or ultimate form). This is a hard-to-reach form, usually only achieved through great hardship.
There's also armor-digivolution, which uses digi-eggs to digivolve further, and DNA-Digivolution, which allows two Digimon to combine together to become one stronger Digimon of the next level, with both conscious. Bio-Digivolution allows the Rookie form of the Digimon to merge souls with their Tamer or Partner to become a Mega level Digimon, with a single consciousness that can feel each's thoughts and feelings. It's said that those forms are stronger than normal Megas.
One thing to note is that in Digimon Tamers, the being which allows Digivolution is a Digimon named Calumon. He was a being called the Catalyst, and was turned into a Digimon. All Digivolutions are powered by his Crystal Matrix power, and without him, Digimon are not able to Digivolve.
